This example was probably generated by the old preprocessing software FUDAA-PREPRO...
This tools allow to define weir in the mesh and automatically generate this file but it was no longer maintained. Maybe it will continue to work but this is not sure...
Actually I would say you should define this file manually, the explaination of the structure of this file is described on the manuals.
Ideally, this could be great to implement such file generation as a module in the Mesh tools plugin of QGIS but this need a contributor to develop or fund

According to the user manual:
A weir must be prepared in the mesh and consists of two boundary lines which are actually linked by the weir. In principle, these boundaries should be sufficiently far apart, upstream and downstream of the weir. The upstream and downstream boundary points should correspond 1 to 1, and the distance between two points should be the same on both sides.
For each weir, it is then necessary to indicate:
The number of points for the first side of the weir (line 5 for the first weir) and the list of their global numbers (line 7 for the first weir). It is necessary to provide the weirs number in the order of the boundary points.
The numbers of their twin points on side 2 should be given on line 9 in the reverse order

What is your misunderstanding with those explanation?

One side is in the order from upstream to downstream, and the other side is the opposite? Is this right? In addition, how should I represent this weir module in the cas file? Take few minutes to have a look at the weirs example of telemac-2D
If you open the mesh (geo_weirs.slf) in BK, display the node number and look at the description file of the weir (fo1_weirs.txt), you will see that the most importatn point is to give the numbers in a coherent order to ensure a direct link between upstream and downstream nodes. There is no impose direction of the flow so upstream and downstream are a convention name.
In the steering file (t2d_weirs.cas) you will also find how to indicate weirs only 2 keywords (WEIRS DATA FILE and NUMBER OF WEIRS)
